I called thinking it was member services for Medicaid. They took name, DOB, debit info, email, and a member ID #. They gave me a verification # and another number to call to track the progress of my new insurance card, but when I called the number it was State Farm...and when I called this original number 866-756-0004 it was a Walmart gift card reward. Pretty sure I got scammed.
